Things that will be required for Kindergarten Registration:
1.) The child must be 5 on or before September 30th, 2021.
2.) Physical w/ shot record and must not be more than 12 months old from the date that student starts school on 7/26/21. Therefore the date of the physical must be between 7/26/20 - 7/26/21. The state of Virginia now requires that all kindergarten students receive the Hepatitis A vaccine (HAV). One dose must be administered before the student may start Kindergarten. See additional information on our school website.
4 DTP With 1 dose being administered on or after 4th birthday
4 POLIO With 1 dose being administered on or after 4th birthday
3 Hepatitis B
2 MMR with 1st dose give at age 12 months or older
2 Hepatitis A (HAV) with the first dose being administered at age 12 months or older
3.) Proof of Residency is required. This must be a currently dated signed lease or a Title or Tax document showing the address of the home owned.
4.) State Issued Birth Certificate
The school system is working towards an online registration process. All of these things will still be required and will be able to be uploaded into the system. More information will follow on this process once this system is online and working.
If you would like to go on and register your student by paper that will be fine but you will need to schedule an appointment between 9:30 am - 11:30 am or 2:00 p.m - 4:00 pm. Even if you do not have all items listed above please go on and register as soon as possible. We are hoping that the online registration will be up and running by the second week of May.
